-If you need to report the death of a loved one, please leave their name and we will return your call as soon as we can.
-In order to process the benefits of member who has died, the MPRA will need a copy of the death certificate sent to our office at the above listed office address.
-After mailing the death certificate to our office it will take some time to process. A beneficiary should receive payment through the mail in a few weeks.
-If you wish to change the names of your beneficiaries, complete the beneficiary change forms and mail them to the above office address.
